Surgical Management of Giant right coronary artery with a right ventricle fistula: a case report

Abstract

A 26-year old male was referred for evaluation of suspected coronary artery fistula. His complaint was shortness of breath on exertion. On physical examination murmur was heard mainly at the level of 4th right intercostal space which radiated vertically. The echocardiogram and coronangiography showed abnormal flow from the aorta to the hollow viscous like chamber. A giant right coronary artery (RCA) with a connection to the right ventricle (RV) was seen on MSCT examination. With Substandard preoperative examination, Intraoperatively, giant RCA was found to be embedded to the myocardial tissue. There was only a small distal branch (less than 1 mm) which hinder anastomosis graft. Fistula outlet was closed from the inside of the right ventricle, and from the distal of giant RCA, further from the distal branch.
