David V Valve Sparing Aortic Root Reimplantation: Experience of Single Center

Abstract
Background: We evaluated the retrospective operative and functional results of David V valve sparing procedure in selected patients.Methods: From April 2009 to January 2017, David V valve-sparing aortic root replacement was performed in 40 patients. Median age was 57 (24-79) years and 30 (75%) were male. Five patients (12,5%) had acute aortic dissection type A and three (7,5%) had bicuspid aortic valve. Additional procedures were performed in 15 patients (37,5%). Aortic insufficiency was defined as trivial, mild, moderate, severe and grades were presented in 2, 20, 13 and 5 patients, respectively. Mean follow-up was 37,4±23,1 months (median 40,5).Results: Concomitant cusp repair was performed in 10 patients (25%) in conjunction with additional cardiac surgeries (37,5%). The follow-up was completed in all, and one patient (2,5%) required aortic valve replacement at early postoperative period for remaining AI. All patients survived surgery, there were 3 hospital deaths; one related to respiratory failure, where as two were caused by low cardiac output syndrome. At follow-up, cerebrovascular event was seen in one and severe aortic regurgitation occurs in one (2,5%) patient. Survival at 1 and 4 years was 93%, 86% respectivelyConclusions: Although the surgical techique of David operation was challenging, it provides excellent midterm effectiveness and low risk of late events.
