Warden’s Procedure versus Single or Two-patch Repair Technique: 15 Years of Long-term Follow-up

Abstract

Objective: to compare the patch technics repair (single and double patch) with the Warden procedure for right partial pulmonary abnormal venous connexion on the sinus rhythm.

Methods: single center retrospective study of all patients with right partial pulmonary abnormal venous connections who underwent a complete repair between 1997 and 2016. A total of 93 patients were included and the mean age at operation was 19 (+ 19) years. Post-operative complications, such as, rhythm disturbances, pacemaker implantation, superior vena cava obstruction and mortality were analyzed and compared between the two groups, the warden procedure group and the patch repair group (single or double patch technic). The Warden procedure group counted 62 (66.7%) patients and the patch technic repair group counted 31 (33.3%) patients, 19 (20,4%) had the single patch procedure and 12 (13%) had the double patch procedure.

Results: the mean follow-up was 5 (+ 4) years. The total mortality was n = 2 (3%), 2 (3%) patients required a pacemaker implantation (both in the Warden group), 2 (3%) suffered from superior vena cava stenosis and 5 (8%) patients had permanent rhythm disturbances. Nevertheless, there weren’t any significant differences between both groups for these criteria (respectively, the p values were 0.3; 0,3; 0,6; 0,1). None of the patient suffered from postoperative pulmonary vein stenosis.

Conclusion: the Warden procedure was known to have better rhythm results according to some previous studies. However in our study the Warden procedure and the patch technic repair showed comparable results concerning rhythm complications, pacemaker implantation and superior vena cava stenosis.
